Use this service to request a repeat prescription.

Allow 2 working days before collecting your prescription.

If medication is requested after 10:30am then this will not be processed until the following day.

You can use this service if you:

• are registered at the surgery

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
